Texas Health Huguley Hospital Fort Worth South has hired Jennifer Young, RN, BSN, MSN as the Transitional Care Coordinator. In this new position, she is responsible for optimizing the transition of care for patients as they are discharged from the hospital.
“In creating this new role, we can provide the best care to our patients beyond our doors. Jennifer and her team will provide additional support and education to our patients, even after they are discharged,” says Tammy Collier, RN, FACHE, vice president of patient care services.
Young will help patients transition from the hospital to home or another health care facility, and work to prevent hospital readmissions. Her role will include educating patients on their diagnosis, discharge instructions and medications and alerting them to conditions which would require medical attention.
“Many patients, especially those recovering from pneumonia, congestive heart failure or heart attacks, are at risk for returning to the hospital. By guiding those patients and their families through discharge and insuring that they understand their condition and have access to their medications, we can reduce the likelihood of readmission,” explains Young.
Young has eight years’ experience as a nurse, most recently as a clinical liaison with Select Medical Corporation.
A resident of Fort Worth, she earned a Bachelor of Science degree in nursing from Texas Christian University. She graduated from Walden University with a Master of Nursing degree.
She is a member of the American Nurses Association, the Texas Nurses Association, and the Case Management Society of DFW.
