Texas Medical Foundation Awards Bronze Star to Huguley Home Health

The Texas Medical Foundation presented Huguley Home Health with the Home Health Collaborative Bronze Star Award.  Based on the two-year time period from January 2005 to December 2007, only 23% of Huguley Home Health patients returned to a hospital upon being discharged from home health care, which is significantly better than the state average.

“Our goal is to educate patients and caregivers on how to adjust and heal at home.  We strive to keep our patients at home where they are most comfortable and can best heal,” said Wiley Whitten, Huguley Home Health Administrator.

The data is based on one of the ten outcomes tracked by Medicare.   This summer, data from the Center for Medicare & Medicaid Services ranked Huguley Home Health higher than the local average on each of the ten outcomes.  The ten outcomes measured are common to patients receiving home health care and include improvement getting to and from the bed, decreased shortness of breath, and increased ability to walk and move.

As part of the Texas Medical Foundation (TMF) Home Health Quality Improvement initiative, TMF provides Texas agencies with tools, resources and evidence-based practices to prevent avoidable hospitalizations and to improve patient self-management of oral medications.  TMF partners with home health professionals to examine their practices and improve systems of providing care.

In October, the 2007 HomeCare Elite™ named Huguley Home Health as one of the nation’s Top 500 home health agencies, placing it in the top 6% in the United States.  Huguley Home Health is the only home health agency in Johnson County or Fort Worth to be awarded this recognition. 

Huguley Home Health agency is a non-profit organization serving patients in Johnson and Tarrant counties since 1984. The agency is Medicare and Medicaid certified and has been accredited by The Joint Commission.  Services available in the home include:  skilled nursing, physical therapy, occupational therapy, speech pathology, home health aides, licensed medical social worker, and chaplain services.
